Nurses provide comprehensive support across various levels of care to ensure patient health and safety.

The role of nurses in today’s healthcare system is far-reaching and indispensable. At Personalized Home Health Care, our nurses acknowledge the essence of their participation in every case they take. Nurses play a substantial part in ensuring patients’ safety and stability during transitional care.

As the presence of physicians decreases in the course of patient recovery in the home care setting, nurses step up to provide the highest level of clinical care available. They carefully monitor patient progress and relay these observations to the clinical team.

Registered Nurses (RNs) and Licensed Vocational Nurses (LVN) work jointly with your doctor to provide home health care services on a one-on-one basis. Their assessment, establishment of a nursing plan, skilled treatment, and support provide the high-quality care you need to recover right at home. A registered nurse is available for admissions, emergencies, or questions and concerns 24 hours a day. The services they provide include:

NURSING CARE ARRANGEMENTS

RNs and LVNs comprise the skilled nursing workforce of Personalized Home Health Care. Through them, we are able to provide various nursing care arrangements that match the level of needs of patients.

Registered Nurse

Registered nurses are the primary professionals who mobilize our skilled nursing services.

Home Health RN
Home Health RNs are trained to develop plans unique for each patient. They work closely with physicians, families, and patients to arrive at the most suitable treatment arrangement. They supervise the activities of LVNs and caregivers and are skilled in administering clinical services in the homes of patients. They provide care on a one-on-one basis but do not stay all in a patient’s home.
Private Duty VIP RN
While home health RNs need to perform administrative care, private duty nurses supplement care for an individual who needs their assistance and monitoring for a specific period of time. Private nursing may occur at home or in a health facility where the patient is recovering.
Travel RN
Travel nurses are home health nurses on the go. They take in a specific number of cases for the day and provide skilled and competent care in the homes of patients. They monitor medical equipment by patients and observe health status. They make the necessary reports regarding a patient’s health progress.

Licensed Vocational Nurse

LVNs enable quality assistive care and support by monitoring patients and attending to their needs.

Home Health LVN
Home health LVNs work alongside or under the supervision of RNs. They ensure the health progress of patients by attending to their basic medical needs. They collect samples, set patients for comfort, and help administer medications.
Private Duty VIP LVN
Just like private duty RNs, private duty LVNs assume their role under the arrangement that they focus on assisting a particular person. As this patient may require close monitoring, specialized care, or long-term care, a private duty LVN is required to stay beside him or her and keep track of the health progress.
